Schedule Your Initial Evaluation
Welcome! To get started with your one-on-one mobile physical therapy as soon as possible, click the button below. Then simply follow the step-by-step instructions to submit your request for an initial evaluation, enter your home address, add your insurance information (if applicable), and include a brief description of your symptoms.
Note: Washington and Idaho are direct-access states, so you can typically begin physical therapy without a physician referral. You can request your initial evaluation today and we will verify whether any referral or prior authorization is required for your insurance plan during the scheduling process.
Step 1: Submit Your Initial Evaluation Request
Click the “Book Your Initial Evaluation” button below. You will be directed to our secure patient portal.
1) Click “Book”

2) Select Initial Evaluation as the appointment type

3) Select Insurance or Self Pay & Preferred Date and Time

4) Briefly describe why you are seeking physical therapy services detailing your symptoms, injury, limitations, weakness, recent surgery, dizziness, etc..)

5) You will then enter your name, email and phone number and click “Create”

6) A 6-digit verification code will be sent to your email. Enter the code and click continue.

7) Your appointment request will be sent but YOU ARE NOT DONE YET. Please click on the “Back to Home” button where you will add your Home Address and Insurance Information (*if applicable)

8) This will take you to your patient portal screen.
Click on Personal to fill in your HOME ADDRESS and all other required boxes with a red asterisk.
Click on Insurance Card Info and upload your INSURANCE CARD information (Primary and secondary/supplemental insurances if applicable) manually or by taking clear pictures of the FRONT & BACK of your card(s)

9) Upon completion both boxes will turn green.
(Self-pay patients may skip the insurance card info step)

Step 2: Louis Calls to Schedule Your Visit
Louis Cerenzia, PT, DPT will personally call you within 1 business day.
During the call, he will verify your insurance benefits (if using insurance), review your location, answer any questions you may have, and will WORK with YOU to SCHEDULE the EARLIEST AVAILABLE APPOINTMENT (as close as possible to your requested date/time) while accommodating our mobile schedule and travel across Pend Oreille and Bonner Counties.
Step 3: Complete Your Digital Intake Forms
Once you and Louis have worked out an Initial Evaluation appointment time that works for you, your appointment will be officially confirmed and you will receive two secure emails from ROAM Physical Therapy — one containing your DIGITAL INTAKE FORM and one containing the PROM (Patient-Reported Outcome Measures) surveys shown below.
These forms are REQUIRED to be COMPLETED before your initial evaluation.

Completing your Intake Form and PROM surveys before your first visit saves you valuable time during the appointment and gives Louis a clear clinical picture of your condition.
The more complete and detailed your information is, the better prepared Louis will be when he arrives. This allows your entire first session to focus fully on hands-on evaluation, personalized treatment, and helping you get results faster — instead of spending time on paperwork.
Please thoroughly complete your:
1) Digital Intake Form, including:
Your home address and phone number (essential for mobile scheduling)
Emergency contact information
Your complete medical history, including current medications, allergies, and past surgeries
Referring physician information (if applicable) and upload any referral notes, recent visit notes, imaging, or surgical reports related to your condition. Note: Washington and Idaho are direct-access states, so a physician referral is not required to begin physical therapy. If you have one, uploading it helps streamline the insurance process. If you do not have a referral and your insurance plan requires one, we will notify you during the scheduling process and assist you in obtaining it.
A detailed description of your symptoms — including where you feel them, what they feel like (pain, numbness, weakness, stiffness, etc.), how long they’ve been present, how they started, what makes them better or worse, and how they affect your daily activities. Please also include the full history of this condition or injury.
Clear photos of the FRONT and BACK of your primary insurance card and any secondary or supplemental cards — you can upload files, take pictures directly in the form, or manually enter the information.
Reading and signing the consent forms, including selecting your payment election (In-Network Insurance, Out-of-Network Insurance, or Self-Pay).
Optional: Consent to securely save a payment card on file for future co-pays, deductibles, or balances. This makes future visits seamless and convenient — no need to handle payment details each time. You will only be charged for services provided and you will always receive a detailed receipt. You can remove or update your card anytime.
2) PROM (Patient-Reported Outcome Measures) Surveys
These short surveys asks specific questions about how your condition affects your daily life and function. Your answers help us better understand your needs and track your progress throughout treatment.
