Insurance and Payment Options

At ROAM, we believe high-quality, one-on-one mobile physical therapy should be accessible. We participate with major insurance plans and offer transparent, flexible payment options so you can choose the path that best fits your needs and budget.

Louis Cerenzia, PT, DPT will personally verify your insurance benefits (including any referral or prior-authorization requirements) during the scheduling process and clearly explain your estimated costs and all available options.

We offer three distinct payment options:

In-Network Insurance

We are proud to be an in-network provider for the following plans:

Premera Blue Cross (including HMO)

• Medicare Part B (Serving Washington & Idaho)

• Federal Employee Program (FEP)

• LifeWise Health Plan of Washington & LifeWise Assurance Company

Under these plans, you are only responsible for your plan’s specific copayments, coinsurance, or deductibles.

Out-of-Network Superbills

If your insurance plan has out-of-network benefits, you can still receive care with us. You will be billed at our Standard Fee Schedule Rates, with payment due at the time of service.

After each session, we will provide you with a detailed Superbill (an itemized medical receipt). You can submit this directly to your insurance company to request reimbursement based on your plan’s out-of-network benefits. While we provide all the necessary coding and documentation, please keep in mind that final reimbursement is not guaranteed and is ultimately determined by your specific insurance provider.

A Good Faith Estimate (GFE) will be provided before your first visit if you’re considering out-of-network benefits. This gives you a clear picture of your expected costs upfront so you can choose the best option for you.

Self-pay & Wellness

We offer simple, straightforward Self-Pay Flat-Fee Bundled Rates for patients who prefer to pay directly and keep things simple. By paying out-of-pocket, you don't have to worry about insurance delays or endless claims paperwork, and your care is based entirely on what you actually need—not on insurance company limits.

We offer two simple options for direct-pay visits:

1. Self-Pay Physical Therapy (Comprehensive Rehabilitation)

For patients who don't have insurance, are out-of-network, or simply choose to pay out-of-pocket to evaluate, diagnose, and treat a specific injury, recent surgery, localized pain, vestibular issue, or medical condition. This is a fully comprehensive track focused on your recovery and healing. It includes every care option available within your state's scope of practice—from hands-on manual therapy and targeted pain relief to full-body strength and conditioning—all carefully tailored and medically prioritized to help you safely return to your active lifestyle and get back to doing what you love.

2. Wellness & Recovery Services (Prevention & Maintenance)

For patients who are already feeling good but want general health optimization, fitness, injury prevention, and long-term vitality. Because these sessions focus entirely on full-body conditioning, longevity, and body maintenance, they do not include a clinical evaluation, medical diagnosis, or treatment plan for an injury or condition. The focus here is on performance, healthy aging, and keeping you moving well.

Every visit is delivered directly to your home or preferred location, is fully customized to your body, and can include:

Balance & Stability Training: Specialized programs to improve your balance, build leg strength, and help you move safely and confidently.

Strength & Conditioning: Personalized exercise programs designed to build everyday strength, support healthy aging, and keep your body resilient.

Dry Needling (Available for Idaho residents only): Trigger point dry needling (including electrical stimulation if needed) to target deep muscle tightness and speed up recovery.

Manual Therapy & Recovery: Joint mobilization, soft tissue work, myofascial release, and trigger point release to ease muscle tension, help you move better, and relieve soreness.

You and your physical therapist will team up at every single visit to choose the perfect mix for that day—whether you need a session focused entirely on hands-on recovery, a great strength workout, or a little bit of both.

Both options are billed at the same flat-fee rate. Sessions are typically 45, 60, or 75 minutes. To keep pricing completely clear and upfront, we provide a Good Faith Estimate (GFE) utilizing these bundled rates to all self-pay, uninsured, and out-of-network patients before the first visit. Payment is due at the time of service.

Important Notes

Your Right to Pay Out-of-Pocket: Under federal law (HIPAA), you have the legal right to pay for your care yourself and ask that we do not share your information or send bills to your health insurance plan. Our Self-Pay PT rates are meant only for uninsured individuals or patients who choose to use this right.

In-Network Insurance Rules: If we are in-network with your specific insurance plan and you want to use your medical benefits for active physical therapy, our insurance contracts require us to bill according to your plan's exact rates and rules rather than our flat cash bundles.

Medicare Beneficiaries: Federal rules do not allow Medicare beneficiaries to pay out-of-pocket for covered, medically necessary physical therapy. We cannot accept private payment for active injury rehab that falls under Medicare Part B. However, Medicare beneficiaries can legally self-pay for our Wellness & Recovery Track, since general fitness, prevention, and conditioning are not covered by Medicare.
