Washington and Idaho are direct access states, meaning you can start physical therapy without a physician referral in most cases. *Certain insurance plans may still require a referral or prior authorization for coverage. 2) Complete Your Digital Intake Form including:

Contact/demographic information (email, phone, address, DOB, etc..)
Past medical and surgical history
A detailed history and explanation of your current symptoms or functional limitations
Primary care physician details
Digital consent forms

Please fill out your Digital Intake Form thoroughly and in its entirety. This allows us to review your case before arrival, create a targeted treatment plan, establish an objective baseline for progress tracking, and avoid administrative delays.

3) Upload Your Documents (within the Digital Intake Form) Including Clear Photos of:

Insurance card(s) (front and back)
Imaging reports or specialist visit notes
Physician referral (if you have one)

No referral? No problem in most cases—just provide your doctor’s name and fax number. We’ll handle coordination, verify benefits, check for referral/prior authorization requirements, and notify you before scheduling.
